This is a grant funded position and subject to the award and continuation of the contract

Here’s What You Need

  • H.S. Diploma or equivalent is required. Successful completion of a Community Health Worker formal training program such as from a community college or other education institution is preferred.
  • Six (6) months customer service experience either face-to-face or by phone is required, bi-lingual a plus.
  • BCLS – Basic Cardiac Life Support is required within 6 months of hire.
  • Six (6) months of public health experience preferred
  • Community health worker certification with the state of Texas is preferred.

Strong verbal and written communication skills, ability to maintain composure in stressful situations, ability to problem solve, time management skills, detail-oriented, ability to work independently as well as with others, strong organizational & prioritization skills, and ability to motivate and inspire others, ability to complete required reports.

 

What You Will Do

Coordinate Community Health Improvement Activities

  • Participate in program development, planning and evaluation.
  • Develop and effectively manage program timelines to ensure timely completion of program deliverables.
  • Monitor program activities and reporting to ensure accuracy in outcome measures.
  • Facilitate small- and large-group education.
  • Establish and manage relationships with community members, organizations and coalitions.
  • Identify strategic partnership opportunities and promote relationship-building.
  • Disseminate programmatic and marketing materials.
  • Develop presentations and other materials for use during program-related events.
  • Serve as programmatic liaison to internal and external stakeholders.
  • Build and maintain collaborative relationships with FQHC and other low-cost clinics to support care coordination 50%
  • Patient Support
  • Responsible for establishing trusting relationships with patients and families.
  • Navigates patients and families through medical and non-medical support services.
  • Conducts eligibility, enrollment and follow up for referred medical or non medical support.
  • Helps patients set personal care plans and attend appointments.
  • Works closely with medical care team to ensure coordinated care.
  • Service as liaison to internal and external resources through TH network.
  • Provides cross coverage support across entities and assisting fellow CHWs to ensure continuity of care.
