Care Management Specialist

Fast-paced, high-volume clinic looking for a top skilled Care Management Specialist

 

 

Work location:  8230 Walnut Hill Lane POB I8390 LBJ Freeway – Suite 1000     Dallas, Texas 75243

Work hours:  Full-time, 40 hours weekly; Monday thru Friday, (8:00am – 5:00pm)

Here’s What You Need:

·       High School Diploma or equivalent (REQUIRED)

·       Four years of work experience in an Outpatient Healthcare Facility (REQUIRED)

·       Healthcare Training Certification (PREFERRED)

·       Customer Service work experience in Insurance or healthcare (PREFERRED)

·       CMA (STRONGLY PREFERRED)

·       Community Health Worker Certification (PREFERRED)

 

 

What You Will Do:

·       Educates and guides patients to understand the benefits and requirements of care management services as needed

·       Receives Chronic Care Management and Remote Physiological Monitoring orders from Neurology Physicians, accesses patient charts and imports information into Care Management dashboard systems (Healtharc, Prevounce, Allscripts)

·       Engage patients by phone, text, or video to: Remind patients to record their vitals; respond to alerts related to patient symptoms or physiologic data; respond to patients for minor technical troubleshooting

·       Works with patients on determining insurance eligibility and authorization requirements for care management support

·       Assists patients with coordination of appointments and other care management needs related to their chronic conditions
