Mobile Health Patient Care Coordinator

 Here’s What You Need:

• Education: High School Diploma or Equivalent
• Experience: 3 years in a healthcare setting with direct patient contact or patient navigation (preferably in primary care).

Certifications:

• Certified Medical Assistant (CMA) – Required upon hire
• Basic Life Support (BLS) – Within 90 days of hire
• Preferred: Bilingual (English/Spanish) for effective patient communication.

What You Will Do:

• Prepare & Follow Up with Patients: Collect histories, track referrals, confirm prescriptions are picked up, and address care barriers.
• Support Preventative Care: Partner with providers to assist patients in completing screenings and health measures.
• Navigate & Advocate: Serve as a liaison between patients, providers, and healthcare teams while conducting outreach to ensure care adherence.
• Assist with Administrative Duties: Maintain tracking logs, document services, and ensure accurate records in the Electronic Medical Record (EMR) system.
• Manage Clinical Supplies & Testing: Stock mobile units, oversee lab supplies, and process point-of-care tests as needed.
