Mobile Health Patient Care Coordinator
Work location: Texas Health Resources, 1301 Pennsylvania Ave, Fort Worth, TX 76104
Work hours: Full-time, Days
Note: This is a grant funded position, subject to contract renewal.

- Experience: 3 years in a healthcare setting with direct patient contact or patient navigation (preferably in primary care).
Certifications:
- Certified Medical Assistant (CMA) – Required upon hire
- Basic Life Support (BLS) – Within 90 days of hire
- Preferred: Bilingual (English/Spanish) for effective patient communication.
What You Will Do:
- Prepare & Follow Up with Patients: Collect histories, track referrals, confirm prescriptions are picked up, and address care barriers.
- Support Preventative Care: Partner with providers to assist patients in completing screenings and health measures.
- Navigate & Advocate: Serve as a liaison between patients, providers, and healthcare teams while conducting outreach to ensure care adherence.
- Assist with Administrative Duties: Maintain tracking logs, document services, and ensure accurate records in the Electronic Medical Record (EMR) system.
- Manage Clinical Supplies & Testing: Stock mobile units, oversee lab supplies, and process point-of-care tests as needed.
