Work location: Hybrid – remote and in office at 612 E. Lamar Blvd, Arlington, Texas 76011 and Command Center located at Texas Health Dallas

Work hours: Monday – Friday from 8am -5pm

Here’s What You Need

  • Bachelor’s Degree in Nursing require or
  • Bachelor’s Degree in Nutrition, Dietetic, Food Science or Related field required 
  • Master’s Degree in Nutrition, Dietetic, Food Science or Related field preferred
  • 2 years certified diabetes care and education specialist required
  • 3 years clinical nursing experience required or
  • 3 years registered dietitian
  •  CDE/CDCES – Certified Diabetes Educator/Certified Diabetes Care and Education Specialist within 12 months of hire required and
  • CPR – Cardiopulmonary Resuscitation upon hire required 
  •  RN – Registered Nurse upon hire required or
  • LD – Licensed Dietitian upon hire required and
  • RD – Registered Dietitian upon hire required and 
  • CDR – Commission on Dietetic Registration Certification upon hire required

What You Will Do

Patient Education

  • Uses advanced interviewing and assessment techniques to obtain patient biophysical, psychosocial and selfcare education and discharge needs of those patients with diabetes.
  • Integrates appropriate assessment of the patient’s readiness to learn, barriers to learning (illiteracy, language, need for communication aids in the geriatric population, etc.), preferred learning methods, cultural influences, age appropriateness, etc. into the individualized diabetes care plan for the patient.
  •  Educate and assist patients with diabetes through appropriate evaluation and teaching methods.
  • Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services

Leadership 

  •  Participates in system-wide projects/committees/workshops to enhance care delivery and the overall experience of the patient.
  •  Provides stewardship in the utilization of resources; contributes to the operational and strategic planning for the diabetes patient population.
  • Collaborates with multidisciplinary team to improve inpatient diabetes care, education and glycemic management.

Quality and Outcome Management

  •  Monitors patient evaluation and management plans for alignment with national guidelines and best practices; shares results through the Performance Improvement Committee and interdisciplinary team rounds as indicated.
  • Advocates for and communicates about improved quality of care and outcomes for those living with, at risk for, and affected by diabetes and cardiometabolic conditions.
  • Applies business principles, systems practice, and population health management to support achievement of the Quadruple Aim (reduced costs, better outcomes, improved experience, and improved work life for healthcare providers).

Clinical Practice

  • Demonstrates advanced knowledge and clinical diabetes expertise serving as a clinical expert and resource for others.
  •  Utilizes appropriate educational resources that are culturally sensitive and meets the learning needs of patients/families.
  • Partners with the interdisciplinary care team to assist with glycemic management in the inpatient diabetes population.
  • Documents services rendered within the medical record reflecting thorough and timely record keeping same day as service provided.
  • Develops relationships with varied community agencies, healthcare resources and physician providers to support the varied needs of the identified diabetes patient population.
  • Is actively engaged in community events affiliated with THR as needed to facilitate visibility and health promotion related to the management of diabetes.
