·        Outpatient Rehab Case Management department

  • Work hours: Full-time, 40 hours/week, Monday-Friday 8:30AM-5:00PM, no weekends and no federal holidays.

·        Providing support to the Outpatient Rehab department with case management

Here’s What You Need

        Bachelor’s degree in social work/counseling, Education, Physical/Occupational/Speech Therapy, Nursing or related field required.

        3 years case management experience with 1 year in rehabilitation field Required.

        BCLS – Basic Cardiac Life Support prior to providing independent patient care and maintained quarterly Required

        Intermediate computer proficiency.

        Ability to give instruction and clearly explain problems and solutions. Strong customer service and problem resolution skills.

        Good organizational and time management skills.

        Must be able to read, write, speak, and understand English instructions, safety rules and guidelines.

 

What You Will Do

        Performs age-appropriate patient assessment and establishing Plan of Care

        Receives patient referral and coordinates information between physicians and therapists to facilitate determination of patient’s appropriateness for the outpatient rehab program.

        Coordinates interdisciplinary team process and care of patient that facilitates comprehensive age-appropriate assessments of
patient’s needs, abilities, and functional limitations.

        Ensure patient / family participation in goal setting and discharge planning.

        Initiates therapy discharge planning in collaboration with care team, patient, and family/significant other based on prognosis and expected discharge dispositions.

        Performs age-appropriate patient treatment.

        Coordinates scheduling of multidisciplinary rehab therapy appointments.

        Performs treatment procedures that are within the scope of the professional license.

        Determine patient response to treatment and evaluate progress (reassessment).

        Educate patients and family through instruction, demonstration, or appropriate handouts in treatment plan, safety issues, home programs/self-management and assistive equipment.

        Communicates with physicians and interdisciplinary care team of the admission assessment process, plan of care, progress, discharge planning, after care needs and other special issues as they arise.

        Utilizes available resources (equipment, support personnel, educational resources) to maximize efficiency and productivity and meet developmental needs.

        Demonstrates commitment to the maintenance of a clean, safe work environment that is compliant with departmental, hospital and regulatory agency requirements.

        Documents all aspects of assessment and plan of care, intake process and treatment sessions in accordance with departmental, TJC, Medicare, and other applicable regulatory guidelines.

        Completes discharge summary for each patient.

        Conduct off campus site visits to establish/maintain relationships and market external referral sources.
