Care Transition Specialist (CTS)

· Work hours: Full-time, 40 hours/week; Monday to Friday Between 7:00AM-4:00PM, 8-hour days, start time will vary – either at 7:00AM or 7:30AM

Here’s What You Need
· High School Diploma or Equivalent Required
· 3 Years relevant experience Required, (preferably in a healthcare or service environment)
· Knowledge of medical terminology preferred
- Problem solving skills
- Latitude for decision making and setting of priorities
· Interpersonal skills and Communication skills
· Ability to work with minimal supervision or without supervision when necessary
· Proficient computer skills with experience in: MS Outlook, MS Office (Word, Excel), internet navigation, clinically based software, and financially based software
What You Will Do
• Responsible for coordinating the clerical and administrative processes for entity care transitions management
• Responsible for managing departmental mailbox, phone lines and/or fax
• Supports transition planning activities as needed under the direction of care transition managers: Calls vendors, physician offices, clinics, etc.; Faxes (electronic and traditional) documents as needed
• Schedule follow-up appointments, call Insurance companies for referral approvals for patients
• Assist/support the Case Managers – will be assigned to 3 to 4 floors of patients – will go into the patient’s rooms for signatures on documents, as well as deliver the Durable Medical Equipment/DME (ie. Wheelchairs, walkers, wound vacs) to the patients prior to discharge
• Makes copies or gathers other data as needed
• Collects and enters data as needed
• Submits for prior authorizations for transportation and post-acute disposition via insurance portal.
• Completes other tasks and projects as assigned
