Here’s What You Need
- Graduate of an accredited LVN program required
- 2 years LVN experience within the last 2 years required or
- 1 year transfer center experience within the last year required
- 1 year THR transfer center experience within the last year preferred
- LVN in the state of Texas upon hire required
What You Will Do
- Coordinates and facilitates patient transfers following the Transfer Center’s process flow and higher level of care requirements
- Collects clinical information to determine patient’s level of care
- Collaborates with THR on-call physicians and non-THR physicians to arrange transfer
- Maintain knowledge base regarding ever evolving changes at each entity
- Utilizes established protocols and guidelines for ensuring patients are safely and efficiently transferred to the appropriate level of care
