Congratulations and welcome to our Texas Health family! You will meet with Employee Health prior to beginning your position at Texas Health. As part of that meeting, you will need to review the items below as part of your pre-boarding journey to ensure you stay on track!

Setup a ReadySet account

This is an important step because you will not be able to complete any of the other steps without this one out of the way first! Please be sure to complete all the Health Assessment Surveys. You are finished when you see a green “completed” next to each survey status.

To Create a username and password for your secure and private My Health webpage. Go to https://texashealth.readysetsecure.com and click “Create a new account” and follow the instructions. You will need the following information:

  • Organization 4-digit Code: 1107
  • Program Type: Post Offer Exam
  • Employee Id: Full date of birth MMDDYYYY- with no dashes or slashes
  • Population Type: Post Offer

If you were previously employed with Texas Health since 2013, please DO NOT create a new account in Readyset. You will need to login to your old account for your surveys. Please contact Employee Health at 682-236-5599 for this information.

Self-Scheduling for Employee Health appointment

After you complete your surveys, the next step is to make your appointment. Please read these considerations first, before you make an appointment.

  • Please schedule your Employee Health appointment no later than 12 days before your start date.
  • Do not make multiple appointments. If you need to reschedule, use the reschedule process below.
  • Do not make an appointment more than 30 days in advance.
  • To schedule an appointment: Select appointment > schedule appointment > type- choose post-offer.
  • Select the location for your appointment. It is preferred that you choose the clinic location at the hospital where you are going to work. If you have a need to attend a different location, please choose the location that fits that need.
  • Select the date and time.
    • Do not make an appointment less than 24 hours in advance
    • Do not make multiple appointments.
    • Do not make an appointment more than 30 days in advance.
  • Select schedule
  • Select OK

    • Select appointments
    • Select the appointment that you wish to reschedule
    • Select reschedule

  • Please remember to arrive 15 minutes prior to your scheduled appointment time and bring the document(s) for the completion of the required I-9 Form. List of acceptable documents can be found here.

Uploading of Immunization and other health-related documents

Please follow these steps to attach your shot records/immunization documents for Employee Health to review.

  • Select documents > upload document > select a file > browse for the file and click the document
  • Select open > window pops up.
    • Document type- choose other.
    • Document name – type a one or two-word description or leave as “other”
  • Select upload.

Please do your best to find and attach a copy of your immunization (you may bring to apt if necessary). Please include any of the following:

  • Measles, Mumps, Rubella vaccinations x2 (or lab results documenting immunity)
  • Varicella vaccinations x2 (or lab results documenting immunity)
  • Tetanus, Diphtheria, and Pertussis (Tdap) vaccination since 2005
  • Hepatitis B vaccinations and titer
  • Current Flu vaccination
  • COVID-19 vaccination
  • Last 2 TB skin tests, TB Blood Test, or documentation of past positive with last Chest X-ray. You must be available to return two days following your scheduled appointment for your TB reading if applicable.

If you require an exemption from any of these vaccines, you will need to complete a Vaccine Exemption Form and bring it with you to your appointment.

What to expect at your appointment?

Review your Immunization records

Provide a Urine Drug Screen

You will need a driver’s license or State ID to verify your identity. In preparation for the screening, make sure you drink fluids, but no more than 16 ounces of fluid in the 3 hours prior to your appointment time to prevent a dilute sample.

Verification of I-9 documents

Please remember to bring your I9 documents to your employee health appointment. A list of acceptable documents can be found here.

You may be fitted with an N95 mask

Complete a Workability Assessment

This will include a color vision test and physical assessment, you may be asked to bend, twist, kneel, squat and a complete core test challenge. Please dress appropriately.
