Texas Health Southwest has the opportunity for a part time, 24 hour per week,RN Community Health/Faith Community educator. Varying hours and days, primarily Mon through Friday with some weekend coverage

Bachelors Degree in Nursing required, Master Degree in nursing, theology, social work or related field preferred.

Minimum three (3) years community development experience and/or teaching in the community.

RN License required upon hire, Faith Community Nurse certification preferred upon hire.

-Promote a health programming with employers, community partners and faith communities in conjunction with
faith community nursing at THR system.
-Identify community partners, employers and faith communities that are interested in furthering health initiatives assisting them in participating in health programming and education to reduce health disparities in the population
-Work collaboratively with the FCN Director to design health outreach for the congregations in the service area.
-Organize selected employers, faith communities, and community partners for the purpose of: developing resources, collaborative networks, assessment of ongoing needs, assist in setting priorities, and provide leadership
to assist in the development health and wellness programs within their faith community.
-Educate employers, community partners and faith communities about health prevention by working closely with FCN, Pastoral Care and Community Outreach and other Nursing Divisions.
-Provide education and special project support for infection prevention and general safety for patients, visitors, staff, and the community

-Design health and wellness needs assessments for employers and community partners.
-Consult with partners about how to develop and sustain a Community Health program that is appropriate for their setting that will aid in health promotion and prevention.
-Link partners with speakers and leaders to address a menu of health topics and programs.
- Identify the best source vendors or authorities to provide screenings, programs, educational materials, consultants, and services/equipment to assist in the development of Community Health.
- Work at developing outside resources such as the American Health Association, American Cancer Association, etc. for training, education and support.
-Assist partners in developing partnerships with each other and with other organizations in the community for the benefit of health promotion and prevention.

- Establish a set of goals with direct outcomes that can be measured to demonstrate the quantitative as well as the qualitative effectiveness of the FCH Program in the local community.
-Track and submit outcome statistics to monitor the effectiveness of the FCH Program on the community.
-Record the number participants and all applicable statistics to show good outcomes.
-Track the number of speaking engagements, educational events, screenings, and referrals.
-Track and report the results of goals and outcomes that are delivered through the FCH Program.
