Our Texas Health Presbyterian Denton location is seeking to hire a full time Program Coordinator Trauma Administration.

Day Shift, Mon-Fri, Occasional weekend or night.

Required Education:
Bachelors Degree in Nursing

Preferred Education:
Masters Degree Nursing or healthcare related field

Required Experience:
5 years Medical/Surgical-Required

Preferred Experience:
3 years Critical Care-Preferred

Required License/Certifications:
RN - Registered Nurse Upon Hire

Preferred License/Certification:
Specialty Certification

Provides leadership and vision for the Heart Failure Program in terms of operations, services and patient care, consistent with best practices and evidence based care. Facilitates the linkage between strategic business lines and provision of an effective and efficient Heart Failure Program.
Leads and directs achievement in the provision of quality care to the identified patient population, working with appropriate staff and physicians. Analyzes data and coordinates implementation of practice changes as needed. Maintains related database.
Collaborates with Chest Pain Coordinator to develop plans for monitoring Heart Failure patient population, including ACS, chest pain, AMI patients. Reviews medical records against pre-established criteria for core measure quality initiatives and other reviews as needed in an accurate and timely manner, meeting assigned deadlines. Enters raw data into various electronic databases and retrieves data in a format that provides information. Communicates the findings of case reviews and abstractions. Analyzes data and suggests opportunities to improve the delivery of patient care. Researches and produces internal and external
organizational benchmarking reports and studies which will assist identification and measurement of continuous improvement priorities and targets as needed/assigned. Functions as work out facilitator for Heart Failure related initiatives.
Develops and maintains collaborative partnerships with physicians, staff and peers to support the implementation, and on-going evaluation of care delivery. Oversees and facilitates care and follow-up of patients. Coordinates education of staff on AMI/
ACS/ Chest Pain related standards.
Coordinates and facilitates provision of educational opportunities for staff, physicians, patients, external providers of care, and the community. Identifies relevant areas, initiates projects and participates on performance improvement teams.
Leads and facilitates involvement of the staff nurses to ensure compliance of core measures for patients.
Facilitates focus groups based on core measure compliance. Advances designated KPIs and other initiatives through multidisciplinary teams. Develops and manages project plans that reflect evidence based practices and leads project teams to accomplish defined goals or improved outcomes . Facilitates performance improvement initiatives
to improve hospital processes. Performs patient safety rounds and reports appropriate findings. Acts as a performance improvement consultant for assigned departments. Completes PDSA on
processes for improvement.
