Texas Health Resources is hiring for a Care Coordination Supervisor for the Southwest Zone (Fort Worth, Arlington, Benbrook and Azle area). The supervisor will travel throughout the southwest side conducting ride along and clinic visits.

Experience required: 3 Years healthcare related experience to include experience in managed care and/or physician practice setting, experience in and expert knowledge of continuum of care (transitions of care) and one year of experience in accountable care organization and/or risk adjustment programs

The Care Coordination zone supervisor will be responsible for the daily operations management of integrated care coordination zone team consisting of licensed and non-licensed employees. The role provides administrative, technical and clinical management of assigned staff responsible for hiring, performance evaluation, performance improvement, education, staff development, work allocation and problem resolution decisions while ensuring the alignment and support of SWHR

PHSC Accountable Care.
Performance Improvement Management
•The supervisor analyzes the program for completeness of tasks, process variances, and day-to-day operations standardization.

Clinic Support
Attend practice manager meetings providing program updates and education
•Build practice team relationships by conducting on-site practice engagement and care coordination needs assessments.
•Trend practices utilization of care coordination services improving areas of underuse and/or gaps in services to advance Care Coordination’s goal for achievement of ACO goals.

Minimum education: Bachelor's Degree in Nursing.
License: Registered Nurse

1 year Supervisory experience or 1 year experience in a leadership role (preceptor, process improvement facilitator, clinical educator) required

Knowledge of Care Coordination, Discharge Planning, and/or Case Management within an ACO Environment.

Knowledge of the principles and methods to restore or enhance social, psycho-social or bio-psycho-social functioning of individuals, couples, families, groups, organizations and communities.

Interpersonal skills to interact with a wide range of constituencies, verbal and written communication skills.

Knowledge of criteria for continuum of care levels of care and services
