The Care Management Specialist (CMSS), also functioning as a Certified Medical Assistant by phone, is responsible for providing direct support to the Care Coordination's Care Manager (CM) Team for administrative and telephonic patient outreach. The CMSS provides patient outreach for care plan compliance as determined by the CM and/or social worker (SW). The CMSS supports through an integrated approach to care management and community outreach providing improved access.

Monday-Friday 1st Shift; primarily 8am-5pm
High School diploma or equivalent required
Three years of experience in a PCP or specialist setting required
Certified Medical Assistant (CMA) required
Experience with a medical management application and the ability to work effectively with multiple computer programs.(Flow Cast, EMR, Sales Force, etc.)
Knowledge of Allscripts and/or Epic, preferred
Proficient in Microsoft Office Applications
Excellent communication, customer service, and phone skills, as well as the ability to build trust and credibility while working independently
Ability to communicate clearly with team members, colleagues, physicians, and clinical staff
Ability to prioritize task by importance delivering assignments in a timely manner
Responsible for providing telephonic patient support as directed by CM or SW for the following:
CM and SW care plan compliance maintenance telephonic outreach
Monitor high risk patient’s scheduled appointments contacting patient same day for “no show or cancelled” rescheduling immediately
Provide outreach and scheduling for renewal medications and associated needed test/lab
Responsible for timely follow-up activity with patients
Work cooperatively with other clinical personnel assigned to the same patient
Performs other duties as assigned by director, SW or CM
Document all encounters with patient in EMR and Salesforce and/or enters all referral activity into the computer system according to THPG policy and procedure.
Responsible for direct Administrative Care Manager Support working under the supervision of licensed staff and assist these professionals with their daily tasks
Retrieve notes from specialist and scan into the EMR
Retrieves non-THR inpatient records
Prepares daily list of patients that have appointments for the day for each CM
Send Letters to patients per CM
Send timely correspondence to appropriate CM/SW regarding any barriers or problems resulting in the referral/task not being complete and/or a result of a crisis or identified patient need due to their outreach call
Contact assigned UHC-AARP members that have never been into the office for PCP engagement and needed quality measures/gaps in care
Completes all administrative functions associated with referral activities in a timely manner.
