The Director, Community Health Improvement (CHI) will be responsible for execution of Impact Circle  strategy across the system and will manage regional managers assigned to support Impact Circles and CHI strategy. Directors will coordinate between multiple stakeholders and execute effective meetings in support of CHI efforts. Will be responsible for developing Impact Circle annual plan that aligns with CHI strategy and supports CHI efforts. Directors will support research of local communities, help build strategic relations in communities and will coordinate and implement CHI efforts across their territory.


Education/Experience: Bachelor's Degree in Public Health, Health Administration, Nursing or relevant field and seven years of experience in grant making or fundraising is required;


Master's degree in Public Health, Health Administration, Nursing or relevant field and five years of experience in grant making or fundraising is required.

**Experience working with executives is preferred**

Essential Functions:
• Facilitate internal Subject Matter Experts (SMEs) to support annual plan, support senior leadership to build community partnerships that align with CHI strategy, and coordinate meeting schedules and logistics as well as vendor/stakeholder to conduct community needs analysis.

• Draft annual plan in collaboration with Impact Circles, identify best practices that can be funded, support fundraising efforts spearheaded by THR Foundation, broaden scope of community network for collaboration to support Zone 2.0 leveraging THR assets and non-traditional partners (employers, etc.).

• Oversight of CHI community programs executed by local managers, data collection and analysis, reporting.

Skills and Abilities:  Experience leading work teams and operationalizing complex healthcare programs, grants management and development experience required, ability to demonstrate excellent judgment, attention to detail, collaborates with various positions and personnel levels within a healthcare and community setting. Strong communication and presentation skills, ability to effectively interface, influence and build alignment with individuals and multi-disciplinary teams and all levels of management; comfortable with working in a dynamic environment, managing multiple projects.
