Our System Services department is seeking to hire a Clinical Outcomes Recognition Registered Nurse full time to support our Texas Health Organization Physician Office.

This position is a member of a collaborative team, who works directly with healthcare providers and practice managers to assist them to achieve national, regional and local recognition. This position will require specific expertise and knowledge on available recognition opportunities and provide clinical expertise related to clinical protocols, evidence based research and quality improvement.

Required Education:
Bachelors Degree, Nursing, business, or related

Required Experience:
2 Years Healthcare quality experience, working with national recognitions programs.

Advanced knowledge of Microsoft Office Suite 2010(Word, PPT, Excel, etc) or newer EPIC, Allscripts EMR's.
• Knowledge of industry standards including but not limited to NCQA, CMS, NQF, and AHRQ. Knowledge of performance
improvement processes, change facilitation and synergy principles.
• Possess strong analytic and technical skills with the ability to translate data into useable information as well as identify and
assess problems.
• Organizational and time management skills to manage multiple priorities simultaneously and respond to changing priorities.
• Excellent verbal and written communication skills; outstanding interpersonal skills; the ability to relate positively with
individuals at all levels both internally and externally.
• Accountable for delivering high quality work with a commitment to service excellence/

Perform and manage tasks in all phases of the individual recognitions process in accordance with the established process map to mentor and assist providers to achieve national, regional and local recognitions. This will include monitoring and tracking available data for individual provider submission to NCQA, working with practice managers and providers to submit for national, regional and local recognitions, communicate and work with providers and practice managers on strengths and opportunities to improve.
Maintain knowledge of NCQA rules and guidelines for individual recognitions programs to educate providers and practices managers on the guidelines and requirements.

Perform and manage tasks in all phases of the practice recognitions process in accordance with the established process map to mentor and lead practices in effective practice clinical transformation. Serve as a lead contact to assigned clinics achieve practice and entity level recognitions such as document collection, gap analysis, tracking, evaluating, recommending opportunities for improvement and conducting post recognition assessment with practices. Provide effective communication to practices and assist with coordination and teaching of Medical Home learning collaboratives (Primary and Specialty)
Maintain knowledge of NCQA rules and guidelines for practice recognitions programs to educate providers and practices managers on the guidelines and requirements

Will participate in quality improvement initiatives and provide clinical expertise to develop clinical recommendations for evidence based quality metrics, clinical protocols, clinical workflows, research and education.

Collaborate with medical director and other team members to research opportunities for recognition of our providers and practices.

Collaborate with manager to help facilitate Quality initiatives and continuously improve by utilizing clinical expertise, critical thinking and problem solving skills to facilitate change management and performance improvement internally and externally and complete other duties as assigned.
