Texas Health Specialty Hospital, a 10-bed long term acute care facility, has a PRN opportunity for a Case Manager RN.  

Case Management in our LTAC is heavily focused on reviewing potential admissions against set admission criteria [intake], and working with a great team, to determine if the patients are appropriate for our facility. This role is not directly responsible for setting our patients up for success in their discharge planning, but will work collaboratively with the Social Worker responsible for discharge planning. The successful candidate will also need to be adept at establishing and maintaining positive working relationships with referral sources [physicians & hospital case managers], and payer case managers. A vital part of the position is keeping  payer Case Managers updated, and make certain we have current authorizations for the patient’s admission.  Being successful in the role is a result of being proactive and driven by the need to drive improvement in your areas of responsibility. Qualifications are as follows:

***Associate's Degree in Nursing is required
***Bachelor of Science in Nursing (BSN) is preferred

***RN-Registered Nurse- license to practice in the state of Texas
***BCCMN-Board Certified Nursing Case Mgmt-CPUR, CPHQ, CCS, PCUM is preferred

***Three (3) years in an acute care setting, case management or utilization review is required
***One (1) year Case Management experience is preferred
***One (1) year experience as an LTAC Coordinator or Clinical Liaison is preferred

- Demonstrated knowledge of Case Management
- Competency in addressing cost and quality issues related to patient care
- Understanding of care integration and coordination.
- Familiarity with these programs –EPIC [electronic health record], Milliman, Invision, & Canopy
- Proficiency with computers to include Microsoft Office suite & Internet use
-Ability to interact professionally and pleasantly with awareness of vital role in maintaining strong referral relationships
- Critical thinking skills & demonstrated use of decision making ability

-Nurtures positive relationships with all referral and payer sources
-Works effectively with the rest of the team in our weekly patient care conference, and when raising awareness of imminent need for patient’s discharge
- Keep payer case manager’s updated on patient progress and maintain current authorizations for patient admissions
- Responsible for intake and ongoing utilization review processes - including preparation of data/graphs/reports
- Follow Milliman criteria for admission and continued stay determinations
- Conduct research and assist as needed with the coding process
- Responsible for interacting proactively with the our Finance department and the Central Business Office on financial matters
-Maintain effective communication strategies to keep internal staff aware of potential admissions and other relevant information
- Ability to work collaboratively and resolve conflict
-Stay current on changes in payer contracts and reflect these changes into your work flow
- Potential for additional chart audits and preparation of data/graphs/reports [small percentage of duties]
