Care Coordinator Emergency Department Specialist to support the Business Growth Strategy department.
This position will be responsible for scheduling physician follow up appointments for patients discharged home from THR’s wholly owned hospitals. This person will also be responsible for coordinating appointments for ER patients who do not have a documented primary care physician. The goal is to schedule follow up appointments prior to patient leaving the hospital.

High School Diploma or equivalent required
Three years demonstrated experience working in a customer service role required
Strong customer service skills
Advanced office software applications skills
Basic understanding of medical terminology
Basic understanding of health insurance benefits
Ability to give instructions and explain any type of problem or situation
Excellent verbal, written communication and interpersonal skills
Good organizational skills
Self-starter with lots of initiative
Ability to plan, organize and manage resources within prescribed time frames (prioritize and focus)
Ability and willingness to learn new software as needed to ensure efficiency and effectiveness
Using appropriate scheduling software, coordinate continuum of care for patient, contingent on insurance status, follow up physician availability and ED physician order.
Obtain all orders placed during schedulers non-working hours and follow protocol to schedule these patients retroactively.
Review ER physician order for follow up care to determine the specialty and desired time frame of the appointment.
Meet with ER patient and/or family member to offer scheduling service and obtain patient availability.
Document all appointments scheduled in appropriate data base.
Continuous contact with physician offices to determine patient status and to gather feedback on process.
