MINIMUM EDUCATION REQUIRED: Bachelor of Physical Therapy

MINIMUM EXPERIENCE REQUIRED: None

MINIMUM LICENSURE / CERTIFICATIONS REQUIRED: Active Texas Physical Therapy License; BCLS within one (1) month hire

MINIMUM REQUIRED SKILLS: Good verbal and written communication skills in English: problem-solving ability; ability to teach others; time management; good interpersonal and customer service skills.

PREFERRED SKILLS, KNOWLEDGE, EXPERIENCE: Strong general-population PT skills experience preferred


WORK HOURS/SCHEDULE: PRN (As Needed), need weekday coverage for last-minute sick, call-ins, holidays and vacations

DUTIES/RESPONSIBILITIES:

Performs age appropriate patient assessment and establishes plan of care
• Obtain/verify physician order for therapy and validate for appropriateness
• Review available medical records and obtain pertinent medical history from patient or family
• Ensure that patient has given informed consent to treatment
• Evaluate each patient using age and diagnosis appropriate assessment tests and measurements
• Integrate assessment results to identify functional problems and prioritize care needs with patient and family
• Sets appropriate measurable, functional goals with time frames to address the identified problems including patient/family goals and preferences
• Determine plan for care, or appropriate therapeutic intervention to achieve stated goals
• Initiate therapy discharge planning in collaboration with care team and patient based on prognosis and expected discharge disposition
• Demonstrates competence in the performance of age appropriate patient assessment and individualizes care based on age specific needs.
Patient goals achieved within established timeframes
• Patient/family demonstrates understanding of treatment
• Equipment operated in safe manner
• Emergency situations responded to efficiently and effectively
• Appropriate infection control measures used with each patient
• Uses proper hand washing techniques according to policy: hand hygiene guidelines
• Two patient identifiers used when providing patient care
• Maintains acceptable level of customer satisfaction from patients, physicians, coworkers and other departments
• Cultural/religious beliefs are considered in delivery of care. Identifies physiologic and developmental normal for patient’s age
• Speaks distinctly if patient has hearing loss
• Neonates: provide appropriate developmental interventions to facilitate participation of infant and family
• Adult-provides information and instruction at a level of understanding appropriate to comprehension level of the patient
• Adolescents-encourages questions regarding fears, included in care decisions with involvement of parents
• Elderly-provide instructions for safe daily activities

• Employee participates in department, hospital, system or community sponsored events
