This position implements the THPG internal monitoring program, which is population health based and works to improve patient outcomes through monitoring, analysis and trending based on accountable metrics and evidence based guidelines. THPG internal monitoring supports individual providers and practices in meeting Quality performance indicators that aid in achieving NCQA Individual and Practice Level recognitions, Accountable Care contract requirements and internal provider quality incentives.
Compile and distribute timely internal monitoring reports and follow up with clinics to review/develop meaningful action plans.
Gather and analyze appropriate data from various sources, make recommendations and develop reports and supporting documentation describing trending in data/opportunities for improvement, root cause analysis, benchmarking and/or evaluation.
Perform and manage tasks in all phases of internal monitoring process including data collection, data tracking and data analysis to evaluate opportunities for improvement. Utilize the FOCUS PDSA method.
Provide ongoing support, onsite practice training, change management facilitation (including but not limited to process mapping, root cause analysis and action planning) and any additional support needed by THPG clinics in order to meet quality performance metrics.
Implement integration plan of multicultural health initiatives into THPG
Microsoft Office Suite 2010 or newer; Intermediate to advance Excel, EPIC, Allscripts, Next Gen EMR’s
Knowledge of industry standards including but not limited to NCQA, CMS, NQF, and AHRQ
Knowledge of performance improvement process change facilitations and synergy principles
Quality statistical methods and analysis; Six Sigma, Lean or PDSA focus
Possess strong analytic and technical skills with the ability to translate complicated data into useable information as well as identify and assess problems
Organizational and time management skills to manage multiple priorities and respond to changing priorities
Excellent verbal and written communication skills; outstanding interpersonal skills; the ability to relate positively with individuals at all levels both internally and externally
Accountable for delivering high quality work with a commitment to service excellence
Individuals in the position are expected to perform various duties throughout the day and are expected to ‘switch gears’ between various duties and multi-task based on priorities and customer exceptions (executive level, management level, and staff level)
They will utilize critical thinking and problem solving skills to facilitate change management and performance improvement internally and externally with customers
